Trim the ends of the zucchini and cut them in half, length- wise. Next cut them into 1/2 inch think half moons. Place the slices of zucchini in the middle of the parchment paper. Put the fillet on top, and add olive oil, salt and pepper. Top with the dill, and lemon slices. Fold over the parchment paper to cover the salmon. Place the packet on a baking sheet in the oven and bake for 1. Open the packet slowly when done cooking. Enjoy! DINNER OPTION 7. Zucchini Noodles with Scallops and Bacon (Serves 2)1/2 lb petite bay scallops, cleaned and rinsed 3 slices of bacon (nitrate and sugar free)4 medium zucchini, washed and dried. Lemon juice of one small lemon. Olive Oil. Sea salt and ground pepper. Lay the cleaned scallops on a paper towel to dry. Lay another paper towel over the top and press down gently to absorb excess moisture, and set them aside. In a large pan, cook the bacon till crisp. While the bacon is cooking, use a vegetable spiralizer, or julienne peeler to make your zucchini noodle. Set the noodles aside. Once the bacon is cooked, remove it from the pan and set it aside. In the same pan, dump some of the bacon fat, but leave about 2 tsp of it. Bring the heat to medium high and add zucchini noodles, with the garlic powder. Saute the zucchini noodles until they are softened, about 3- 5 minutes. While the noodles are cooking, chop up the bacon pieces. When the noodles are done cooking, remove pan from heat, toss in the bacon pieces , the chopped green onion, and the lemon juice. Add some olive oil to another pan, and set heat to medium- high. Hernia SymptomsWhen a paraesophageal hernia begins to cause symptoms (chest pain, upper abdominal pain, difficulty swallowing), these are usually repaired. Symptomatic paraesophageal hernias are at higher risk for progressing to incarceration (stomach gets stuck resulting in obstruction) or ischemia (blood supply to the stomach is cut off) resulting in the need for emergency surgery. When should a paraesophageal hernia be repaired? In general, all paraesophageal hernias causing symptoms should be repaired. Common symptoms from a paraesophageal hernia include: Chest pain – there are many causes for chest pain. It is important that patients who have a large paraesophageal hernia with chest pain undergo some kind of a cardiac evaluation to make sure that the chest pain is not from their heart. HeniaTypically, eating brings on chest pain from a paraesophageal hernia. Some patients have pain every time they eat, and others only experience discomfort every once in a while. Epigastric pain – this is pain in the middle, upper abdomen. Dysphagia – difficulty swallowing. Shortness of breath – in some very large paraesophageal hernias, the stomach may push on the diaphragm or compress the lungs contributing to a sensation of shortness of breath. There are many other reasons for shortness of breath in addition to a paraesophageal hernia. Stomach ulcer – in some patients with paraesophageal hernias, the stomach may twist upon itself resulting in a specific kind of stomach ulcer known as a Cameron’s erosion. These ulcers can occasionally contribute to chronic slow blood loss and anemia. Many patients (but not all) with paraesophageal hernias may also suffer from gastroesophageal reflux disease symptoms. GERD by itself is not a reason to repair a paraesophageal hernia. GERD is first treated with medications, and surgery is reserved for those who fail medical management. Currently, most paraesophageal hernias can successfully and safely be repaired laparoscopically (with about 5 very small incisions) and through the abdomen (rather than the chest cavity). The laparoscopic repair of large paraesophageal hernias (most of the stomach resides above the diaphragm in the chest cavity) is a complex procedure and should only be attempted by expert laparoscopic surgeons with extensive experience in laparoscopic foregut surgery. During surgery, the stomach is gradually moved back into the abdominal cavity. The diaphragm at the esophageal hiatus is closed to prevent the stomach from re- herniating. In some cases, a special kind of mesh is needed to close the diaphragm appropriately. Once the diaphragm has been closed, most patients undergo a fundoplication or a . The fundoplication is performed to help keep the stomach from herniating back into the chest cavity. What are the results of paraesophageal hernia repair? In the hands of experienced surgeons, the results of laparoscopic paraesophageal hernia repair are excellent. A minimally invasive laparoscopic approach results in significantly fewer complications than an open abdominal approach (many small incisions instead of one large incision in an open approach). Most patients are in the hospital for only 1- 2 days, and are back to their usual activities within 4 weeks. Side effects can occur, and are similar to those observed after laparoscopic Nissen fundoplication. Abdominal bloating can occur, but is rarely severe. Difficulty swallowing (dysphagia) is another side effect that tends to improve in most patients with time – provided food is chewed thoroughly. The majority of patients are able to belch easily when necessary; especially once some time has passed following surgery. As I read the MH article, I was moved by the great insight the author provided as he related the day- to- day lifestyle of an “intermittent- faster”, and describe all eloquently all the ways it had improved his life. I couldn’t help but think the universe was telling me something. This article was no coincidence, it was a sign. I pondered on Ralph Waldo Emerson’s wisdom that “all life is one big experiment. The more experiments you do, the better.” and right there and then, I made a decision: “Intermittent Fasting for 1. Let’s do it.” Boom! I turned off the lights and closed my eyes, hoping to sleep for as long as possible. I wasn’t allowed to “break fast” until 5: 3. I had solid 7. 5 hours of no- food time ahead of me. All righttttt! I thought about it, and couldn’t even recall the last time I’d gone more than an hour before having my morning smoothie. I couldn’t believe how well this experiment was going. When I got back from my bike ride, I remembered I had to do something. It was Sunday 4pm, the weekend market was about to close, and I didn’t have my groceries for the week. That’s right, I had to go grocery shopping. I had to go buying large amounts of delicious food after 1. The stakes had just been raised. I walked into the market, and my nostrils were immediately flooded with the wonderful aromas of fresh baked bread and croissants. I felt this sharp pain in my stomach, and thought about turning around. But no, this was a challenge, and pushing myself through this could only make me stronger. I got everything I needed, paid for my stuff, and even managed to remain courteous to the cashier despite my advance state of hunger. As I walked out, I looked down at my watch. I just had to go home, put the groceries away, then I would cook one hell of a feast. I felt a bit woozy driving home, my mind was a bit foggy as I unpacked my groceries, but before I knew it, my i. Phone alarm went off. A proud smile beamed over my face. I had just crushed this fasting day, put myself out of my comfort zone, and come out a bit stronger on the other side. Best feeling ever. Both are equally important part of getting my mind where I want it to be. Stepping Things Up: Training in a Fasted State. On the 1. 0th Day of Fasting, I celebrated by pushing the envelope a bit.
